About Social Prescribers
We understand that patients visit their GPs for various reasons and sometimes these can be caused by non-medical matters. Our social prescribers work in partnership with the surgery playing a vital role in connecting people to activities, groups and services within the community to address practical, social and emotional needs that impact health and wellbeing.
Our social prescribers can offer tailored support giving you the time to explore and identify what is important for your health and wellbeing and the issues you’d like to address.
They can speak with you on the phone or meet face-to-face. They will work with you to create a personalised plan to help you to connect with services in your community.
At Hanscombe House Surgery we have two Social Prescribers:
For Adult Social Subscribing: Dee Gudgeon
For Children and Young People Social Prescribing: Mary Oshinnaya
If you would like support from a social prescriber ask either your GP or reception team.
Some areas they can help with:
- Accessing specialist services and support
- Struggling with finance and need advice
- Feeling lonely or isolated
- Need help with housing and advice
- Need help managing a life changing event and/or a long-term health condition
- Want to become more active
- Feeling low, or stressed
- Need to talk through multiple things affecting your wellbeing
- Signposting to befriending services, counselling and other support groups and more.
